We are having a problem with the APPROVALLINK token in the notifications. For some users the link has the complete URL for others we only get the relative path. For example:
Full Path:
http://fmhouston.com/MinistriesPrograms/Family.aspx?mid=XXX&ctl=Approval&ContentID=XXXX&uid=X
Relative:
/MinistriesPrograms/Family.aspx?mid=XXX&ctl=Approval&ContentID=XXXX&uid=X
So, in our notification emails, sometimes the link can not be used and must be cut and pasted along with the domain name. Can you think of a reason this would happen?

Do you mean it is still happening? or since when?
Or this error happened after you upgraded to new version?

It is happening right now. We are using Content Manager version 4.2.0 with DNN version 5.2.0. It has actually always been a problem.

I don't have a quick fix for this, we need to investigate on this and fix it if there is a problem.
